Department: Youth Services- DMH Wraparound Services

Reports To: Program Supervisor

POSITION SUMMARY

Individual must have a child or close relative in the system with an emotional disability and have personal life experience with either DMH, Probation or DCFS. personal life experience with either DMH, Probation or DCFS.

Responsible for acting as a parent/caregiver advocate for individuals and families enrolled in DMH Wraparound Program. This program involves the coordination of care and intervention for high-risk children/youth being reunited with their families. The overall goal being to assist the family attain self[1]sufficiency. The Parent Partner reports to the Program Supervisor for administrative activities and the Clinical Supervisor for clinical issues.

SALARY RANGE:

$24.75 - $28.50 per hour DOE

HOURS

Hours vary depending upon availability of the families but requires on-call 24-hour coverage and availability 7 days per week.

CATEGORIES OF DUTIES

1. Typically has a caseload of up to ten (10) clients with the potential to increase to up to fifteen (15) clients at the discretion of the program.

2. Work closely with the child/youth’s parents/caregivers to represent their best interests and participate as an active member of the Child and Family Team.

3. Guide child’s parents/caregivers through the child welfare and/or juvenile justice system.

4. Work closely with the Facilitator to ensure coordination of services available in the community.

5. Attends all required Wraparound Program meetings.

6. Ensures that there is continuous input and feedback from the family and services providers.

7. Cooperates and communicates fully with other staff members regarding patient behavior, progress, activities, etc.

8. Conducts self in a professional manner in areas of dress, language, attitude, and behavior.

9. Exhibits full knowledge and support of the philosophy and objectives of the program and agency.

10. Fully understands and maintains policies regarding professional ethics, including appropriate boundaries and patient confidentiality.

11. Provides proper documentation in the electronic health record of services rendered.

12. Participates in medical record reviews.

13. Knowledge of current legal standards pertaining to professional practice, reporting requirements (i.e., suspected abuse; Tarasoff), professional boundaries, and related issues.

14. Assists in conducting performance evaluations of staff, as required.

15. Additional functions as specifically requested by the Program Supervisor, Clinical Supervisor, Program Director of Youth Services, and Administration.

EDUCATION/EXPERIENCE/QUALIFICATIONS

  • Individual must have a child or close relative in the child welfare or juvenile justice system with an emotional disability and have personal life experience with either DMH, Probation or DCFS.
  • High school diploma or equivalent
  • Medi-Cal Peer Support Specialist Certification (PSCC)
