POSITION SUMMARY

Provides case management services to patients assigned to individual caseload.

Salary Range

$25.75 - $29.50 / Hour DOE

HOURS

8 per Day / 5 Days per Week

CATEGORIES OF DUTIES

General Duties:

  1. Conducts self in a professional manner in areas of dress, language, attitude and behavior.
  2. Exhibits full knowledge and support of the philosophy and objectives of the Agency.

  4. Inform patients about other services offered by Tarzana Treatment Centers, Inc. (TTC) that may be appropriate for the patient’s family members.
  5. Attends in-services trainings and educational workshops.
  6. Attends all required staff meetings.
  7. Cooperates and communicates fully with other staff members regarding patient behavior, progress activities, etc.
  8. As appropriate works closely with other multi-disciplinary team members to develop a comprehensive Individualized Treatment Plan on assigned patients and to ensure that all identified problems are addressed.
  9. Assist with employment/vocational referrals, provide vocational counseling re resume preparation, job interviewing etc.
  10. Provides outreach and follow up services to patients in and out of treatment, to check progress, offer additional services, help reengage in treatment, make needed referrals.
  11. Facilitates discharge-planning efforts, commencing at time of admission on all assigned patients, including areas of housing, employment, medical psychiatric, financial, aftercare, etc.
  12. Makes direct contact within agency and with outside entities as appropriate, in order to assist patients in resolution of legal, financial, housing, medical, psychiatric and discharge planning issues
  13. Provides regular, comprehensive progress note charting in patient’s Medical Record which document contacts, progress towards identified goals, program participation, referrals, discharge planning and other case management activities.
  14. Performs other duties as assigned by Director and/or Administrator.

EXPERIENCE

1. For AOD Programs:

Case Manager I - Completion of an academic Addiction Studies program and certification with ACADC, CCBCDC, BI, FACT, IACC or CAARR

  • Or Bachelors Degree and registration with one of the 10 DADP approved state level certification programs. Must obtain certification within 5 years of employment.
  • 3 year Case Management experience

Case Manager II - Completion of an academic Addiction Studies program and certification with CAADAC, CAADE or AAHCP

  • or Masters Degree in Behavioral Sciences or in at least 3rd year of a doctoral program in Psychology; or Internship registration with the Board of Psychology as a Psychological Assistant or with the Board of Behavioral Sciences.
  • *If not licensed after three (3) years of employment must show evidence of enrollment in Addiction Studies Program and register with one of the DADP-approved state level counselor certification agencies
  • Must be registered or certified upon hire and no later than six (6) months from date of employment.
  • 5 years Case Management experience

2. For all other Programs:

  • Case Manager I - BA/BS/BSW degree in social work or related fields and 3 years prior case management experience
  • AFSD
  • Case Manager II - Masters degree in Behavioral Sciences or in at least 3rd years of a doctoral program in Psychology and 5 years Case Management experience

3. Bilingual English/Spanish preferred

4. Valid California driver’s license and insurance

LICENSES/CERTIFICATES

  • For AOD programs, registration or certification with a DAPP-approved state level counselor certification agencies.

Addendum for HIV Services

Additional Duties:

  1. Performs comprehensive patient assessments and on-going re-assessments during incarceration and post-release.
  2. Develops a comprehensive service plan for each patient with review/revisions as needed, based on patient eligibility and re-assessment.
  3. Provides patients with successful linkage to appropriate community services and maintains follow-up.
  4. Maintains accurate and confidential patient records, including necessary paperwork, assessments, service plans, progress notes, linkages, interventions, referrals, correspondence and phone logs.
  5. Establishes and maintains active working relationships with resources in both the public and private benefits arena and assistant programs and systems.
  6. Establishes and keeps current a resource file/directory of information regarding available programs, services, contact persons and avenues of access in the community and current trends of knowledge regarding HIV/AIDS.
  7. Attends community and agency meetings as required.
  8. Assists with orienting new Case Managers to program’s policies and procedures.
  9. Develops and submits accurate reports regarding program activities, patient’s served, caseload and other information as needed.
  10. Utilizes independence within the institutions or patient’s home when arranging for patient interviews, taking into consideration jail/prison field protocol, safety issues and special circumstances.
  11. Promotes TCMP management throughout LA County, solicits and assess patient’s interest in program participation and assures that patients meet the eligibility criteria as specified by the California Office of AIDS and/or OAPP.

Additional Requirements for HIV Services:

  • Security clearance as appropriate
  • Ability to work independently without daily supervision

Addendum for Home Based Probation Program

Additional Duties:

  1. Meetings with the parents and youth, in person and via telephone.
  2. Assist parents with monitoring techniques of the youth activities at home. Specifically during the hours of 3:00 p.m. to 10:00 p.m. and on weekends.
  3. Facilitate setting weekly goals and assignments for the youth.
  4. Facilitate the youth’s weekly involvement in pro-social activities such as after school enrichment activities, community based recreational programs, employment internships, community/faith based activities.
  5. Establish protocols for parents school involvement including; daily attendance, homework guidelines, weekly progress reports, attendance at teacher/parent conferences.
  6. Develop incentives and rewards for the youth to acknowledge improved school and in-home behavior.
  7. Attends in-services trainings and educational workshops including training regarding the social learning model.
  8. Will report critical incidents to the Program Operation Supervisor or Program Director.
  9. The Case Manager will provide home-based services to approximately 40% or about 22 clients.
  10. Provide service documentation meeting County requirements and maintains confidential files for all assigned participants.
  11. Will communicate with the county assigned case manager for each enrolled participant on a scheduled and contingency basis as needed.
  12. Attends all required staff meetings. Participates in bi-monthly project team meetings and participates in the monthly case conferences involving the participant, parents and/or guardians, the County, and other community stakeholders.


Addendum for FSP Programs

Additional Duties:

  1. Reviews all assessments and any available/appropriate past Medical Records for this or other agencies.
  2. Works closely with other multi-disciplinary team members to develop a comprehensive care plan on assigned patients and to ensure that all identified problems are addressed.
  3. Provides regular, comprehensive progress note charting in patient’s Medical Record which document contacts, progress towards identified goals, program participation, referrals, discharge planning and other case management activities.
  4. Performs individual case-management sessions on a regular basis with assigned patients both in the facility and in the field as appropriate.
  5. Makes direct contact within agency and with outside entities as appropriate, in order to assist patients in resolution of legal, financial, housing, medical, psychiatric and discharge planning issues.
  6. Provides case management services in the field which may also include transporting patients to appointments, etc.
  7. Cooperates and communicates fully with other staff members regarding patient behavior, progress activities, etc.
  8. Is available for Crisis Intervention services as assigned by supervisor
  9. Performs on-call crisis-response duties as scheduled after hours and on weekends.

Additional Requirements for FSP Program:

  • Class B Drivers License
