Position Title: Case Manager I

Intensive Case Management Services Permanent Supportive Housing
POSITION SUMMARY
Provides case management services to patients assigned to individual caseloads.
SALARY RANGE:
$25.75 - $29.50 per hour DOE
HOURS
8 per Day / 5 Days per Week M-F
CATEGORIES OF DUTIES
General Duties:
1. Professionally conducts self in areas of dress, language, attitude, and behavior.
2. Exhibits full knowledge and support of the philosophy and objectives of the Agency.
3. Fully understands and maintains policies regarding professional ethics.
4. Inform patients about other services offered by Tarzana Treatment Centers, Inc. (TTC) that may be appropriate for the patient’s family members.
5. Attends in-service training and educational workshops.
6. Attends all required staff meetings.
7. Cooperates and communicates fully with other staff members regarding patient behavior, progress activities, etc.
8. As appropriate work closely with other multi-disciplinary team members to develop a comprehensive Individualized Treatment Plan on assigned patients and to ensure that all identified problems are addressed.
9. Assist with employment/vocational referrals, and provide vocational counseling resume preparation, job interviewing, etc.
10. Provides outreach and follow-up services to patients in and out of treatment, to check progress, offer additional services, help reengage in treatment, and make needed referrals.
11. Facilitates housing application and placement. Help with employment, medical psychiatric, financial, aftercare, etc.
12. Makes direct contact within the agency and with outside entities as appropriate, to assist patients in the resolution of legal, financial, housing, medical, psychiatric, and discharge planning issues
13. Provides regular, comprehensive progress note charting document contacts, progress towards identified goals, program participation, referrals, discharge planning, and other case management activities.
14. Performs other duties the Director and Administrator assign.

3. Bilingual English/Spanish preferred but not required
4. Valid California driver’s license and insurance
