Position: Patient Navigator I

Seeking full time Patient Navigator for program that will serve unhoused adults accessing health services internally and throughout service area 2 (San Fernando Valley, Glendale, Canoga Park etc.). The Patient Navigator will be work collaboratively with behavioral health staff and community partners to provide Housing & Behavioral Health Navigation services. The ideal candidate will have strong oral and written communication skills and provide excellent customer service that is sensitive to the challenges that homeless persons with a range of medical and behavioral health issues face. Duties will include, conducting behavioral assessments; service planning; linkages to health, mental health, substance use disorder services and other supportive services; crisis management; client education; and housing location services.
Navigators will respond to and schedule patients who contact Tarzana Treatment Centers (TTC) referrals from government and non-profit organizations to include county agencies, hospitals, medical groups, health centers, mental health and substance use treatment providers and others and navigate patients to receive services at Tarzana Treatment Centers.
$25.75 - $29.50 per Hour DOE

Position is responsible for completing preliminary screening to help determine level of care and services to support recovery efforts, as well to help individual access needed health and social support services. Position conducts on-site and/or field- based outreach, engagement, re-engagement and navigation services and scheduling.
Navigation
Conduct initial navigation screening on the phone or in person with the objective to help determining the need and services of each individual and respond to referral sources from organizations to include county agencies, hospitals, medical groups, health centers, mental health and substance use treatment providers and others.
Provide enhanced patient navigation services to identify and link eligible participants to project services.
Participate in outreach efforts at community sites and in-reach at TTC service site.
Maintain accurate and timely information in the Access Log to document all contact and navigation encounters with individuals or referral sources.
Scheduling
Receive warm transfers from navigators and schedule patients for intake via the Electronic Health Record (e.g., Avatar Scheduling Calendar for behavioral health)
Maintain accurate and timely notes to document all attempts at scheduling and encounters with the individual and referral sources in the Access Log and follow up with pending Scheduling Assignments in the Access Log to ensure all scheduled appointments and linkages are completed.
General
Provide excellent customer service, and timely response times during scheduled call center shift.
Provide weekly reports of new individuals enrolled in navigation services to the immediate supervisor.
Provide advocacy as needed to help address issues and barriers between patient and service providers.
Maintain a database of patients served and outcome data for reporting purposes.
Attend agency and community meetings as directed by supervisor.
Execute additional tasks, projects and responsibilities as assigned.
1 or more years of case management experience.
Passion and desire to work with individuals/families.
Demonstrate a non-judgmental attitude and positive regard for families impacted by lack of access to health services.
Experience providing field-based services.
Strong people skills and ability to work with people of various ages, racial, ethnic, cultural, educational and socio-economic backgrounds.
Organized, detail oriented and creative problem-solving.
Proficient in computer skills and use of Microsoft products such as Outlook, Word and Excel.
Must have valid CA driver’s license
High School Diploma or GED
Bilingual English/Spanish preferred
