Health Educator/Outreach Worker I

Seeking a Full-time Health Educator/Outreach Specialist for Substance Use Treatment/Prevention programs. Health Educator will be responsible for the day to day activities associated with health education and outreach to provide and support effective Substance use prevention/education to multiple populations. Responsibilities include conducting outreach and engagement with community partners, facilitating presentations and attending meetings with interested agencies and groups, developing outreach and engagement materials related to SUD treatment/prevention and other duties as directed by the Project Coordinator and Director.

SALARY RANGE:

$25.75 - $29.50 per hour DOE

HOURS

8 per Day / 5 Days per Week

CATEGORIES OF DUTIES

  1. Promotes Health Education throughout the community, solicits and assesses patients’ interest in program participation and assures that patients meet the eligibility criteria as specified by the program objectives.
  2. Conducts structured, scheduled educational sessions covering approved topics.
  3. Identifies site locations for outreach work and health education classes.
  4. Conducts formal event outreach to foster awareness of healthcare services.
  5. Recommends pertinent information and referral services for medical, social and emotional support.
  6. Updates curricula for health education sessions, including pre- and post-tests.
  7. Establishes and keeps current a resource file/directory of information regarding available programs, services, contact persons and avenues of access in the community, and current trends of knowledge regarding health services.
  8. Attends community and agency meetings as well as outreach events as required.
  9. Assists with orienting new Health Educators to program’s policies and procedures.
  10. Develops and submits accurate reports regarding program activities, patients served and other information as needed.
  11. Performs other duties as assigned by Supervisor, Director and/or Administration.

EDUCATION/EXPERIENCE

  • BA/BS degree in health or human/social service-related field preferred or 1 year work related experience.
  • Valid California driver’s license and insurance.
  • Bi-lingual English/Spanish a plus.
